Change of Civil Status Regarding EU Citizens and Their Family Members
All EU citizens holding a registration certificate or a permanent residence permit, their family members, and the citizens of the United Kingdom and their family members under the Brexit withdrawal agreement must report a change of their civil status.
In what case do you have to report a change of civil status?
If your civil status has changed – after you have got married, divorced or after you have become widowed.
What is the time limit to report the change?
You must report a change of civil status within 15 working days. Report the change only after it has taken place.
How and where can you report the change?
You can report the change personally at a Ministry of the Interior office, send it by post, Data Box or by e-mail with an advanced electronic signature, or you can submit it through an authorised representative.
What is the administrative fee?
There is no administrative fee for reporting a change of civil status.
STEPS TO FOLLOW IN ORDER TO REPORT A CHANGE OF CIVIL STATUS:
1. Prepare the necessary documents
In order to report the change you will need:
- Travel document.
- Form for notification of changes.
- Proof of a change of civil status (a marriage certificate, a final judgement on a divorce or a death certificate).

2. Fill in the form for notification of changes
You can fill in the form online. Then print out the filled-in form and sign it.
You can also download a blank form, print it out and fill it in by hand, or you can collect the form free of charge at one of the offices of the Ministry of the Interior. The form must be filled in legibly, in block capitals, Latin characters and in Czech.
3. Send the form or make an appointment at a Ministry of the Interior office
You can report the change personally at a Ministry of the Interior office, send it by post, Data Box or by e-mail with an advanced electronic signature, or you can submit it through an authorised representative.
In the case of a civil status change, you do not need to go to a Ministry of the Interior office in person. You can simply report the change by post.
If you want to report the change by post, you can check whether it has been registered on our hotline. In this case, it is recommended to call the hotline no sooner than a week after the form was sent as it takes a few days to process the incoming post.
If you wish to make the notification in person, please make an appointment online or by phone beforehand. If the offered appointment slot is after your time limit for notification of the change (15 days) expires, send the documents by post, Data Box or by e-mail with an advanced electronic signature, or visit a Ministry of the Interior office during opening hours without an appointment. With no appointment reservation, you have to wait your turn. If you fail to report the change within the time limit, you can be fined.
