If you are applying for a registration certificate, select a purpose of residence that you want to carry out in the Czech Republic in
Section II of the corresponding printed form.
If you are applying for family purposes, you must provide a corresponding proof.
In other cases, you do not have to prove the purpose of your residence in the Czech Republic.
If you are applying for a permanent residence permit, select in Section II of the corresponding printed form a purpose of residence that you want to carry out in the Czech Republic.
If, for the selected purpose of residence, you need to prove the continuousness of your stay in the Czech Republic, prove it, for example, in one of the following ways.
If you must prove employment or self-employment (if you are applying, for instance, for a permanent residence permit under Section 87g (1b)), you can do so, for example, with:
- an employment termination agreement,
- a form with a stamp from the District (Prague) Social Security Administration, or the Trade Licensing Office if you terminated your self-employment,
- an executive pay agreement provided that an extract from a public register clearly shows that you no longer work as an executive etc.
If you must prove for the selected purpose of residence that you meet the eligibility requirements to receive a retirement pension before you have reached retirement age, please provide:
- a decision of the corresponding Social Security Administration to award you a retirement pension before you have reached retirement age.
If you are applying for a permanent residence permit for a child born in the Czech Republic who is an EU citizen, you did not manage to submit the application within 60 days from the date the child was born and you are applying for humanitarian reasons, also attach to the application:
- the child’s birth certificate.
If you must prove disability, do so with:
- a decision of the corresponding public administration authority (a degree of disability cannot be proved with a disability card).
